Your first visit to our office:
If you are a new patient or have not visited our office in more than a year, please arrive 15 minutes early to register and update information. Please come prepared with the following:

Referral, please assure that your referral from your primary physician is in place prior to your appointment, if your plan requires you to have one.

Insurance Card

Co-payment is expected at the time of the visit if required by your insurance.

The name, telephone number and address of your primary care physician.

Any recent records, or diagnostic images that you feel will be helpful to the physician you are seeing.
When scheduling your appointment, you may experience a hold time. Please be patient, frequently we are scheduling a consultation, and procedure appointment, for a patient referred by their primary care physician for a procedure. This does take time. If you cannot hold, there is an option on our voice system for messages. Please leave a message and someone will return a call to you by the end of the day, to schedule a routine non - emergent appointment.
* Please notify our office within a 24 hour period to avoid being assessed a fee. Missed appointments will be billed at $ 50.00 without prior notification. There is a fee of $100.00 for missed procedures.

Directions from the Connecticut
Thruway [I 95] South Bound
Take I-95 to exit 3, Arch Street Exit
Make a right off the ramp onto Arch Street
Then, continue straight on Arch Street. At second light (after passing under green railroad bridge) turn left onto Railroad Avenue.
Remain on Railroad Avenue, which will merge into Hamilton Avenue.
At the forth-light make a Right turn onto St. Roch Avenue.
Continue on St. Roch Ave at the end of St. Roch turn left onto Holly Hill Lane.
You will see our building to your right at you make this turn. Turn right into our parking lot.
This is the back of our building. There is parking on all sides of the building. Our entrance is an exterior entrance in the front of the building before you enter the glass lobby. Suite 100
Directions from the Connecticut
Thruway [I-95] North Bound Take I –-95 to Exit 2, Delevan
Avenue
Make a left off the ramp, proceed to light, and turn right onto Ritch Avenue.
remain on Ritch Avenue, which will merge into Hamilton Avenue. You will go through a stone railroad underpass.
Proceed to second light after underpass, turn Left onto St. Roch Avenue.
Continue on St Roch Ave, at the end of St. Roch; turn left onto Holly Hill Lane.
You will see our building to your right as you make this turn. Turn right into our parking lot.

Local Directions from within Greenwich
From the Greenwich Library, take West Putnam Avenue, South, you will pass Wendy’s Restaurant on your left, continue toward Port Chester. Our building will be on your left right after a large store, Anthropology. If you pass a Citgo Station on your right, you have passed our building. We are directly across the street from the Citgo Station.
Directions from the Merritt Parkway
–South
Take the Merritt Parkway South to Exit 29, Lake Avenue Exit
Bear Right off the exit to stop sign, then right onto Lake Avenue. Veer around small rotary and continue on Lake Avenue.
Stay on Lake Avenue to Lake Avenue Circle, at the circle bear extreme Right onto Glenville Road.
Continue straight on Glenville Road and make second left onto Valley Drive.
Stay on Valley Drive to the light at the intersection of Valley Drive and West Putnam Avenue. Our building will be facing you at the light. Top
